When you contact Ara, let her know your counseling program of choice. Then, download and complete our Nutrition Profile Form [PDF] [Word]. This is a short questionnaire that asks about your nutrition and health goals, your health history and your eating and exercise habits. Once you've completed the Nutrition Profile Form, simply return it via e-mail at least 48 hours prior to your first appointment. This allows Ara to start working on your nutrition counseling program before meeting with you.
